Service Fee Schedule
Initial Evaluation — $350
Your initial evaluation with a nurse practitioner or physician assistant focuses on understanding your mental health needs. They will assess your symptoms, discuss potential treatments, and work with you to create a personalized care plan, which may include medications if needed.
Follow-Up — $200
In a follow-up appointment with a nurse practitioner, you'll review your progress and discuss any changes in symptoms. Your provider may update your treatment plan, adjust medications if needed, and work with you to continue building on your progress.
Direct Pay Fee Schedule
No-Shows / Late Cancellations — $100
Assessed for missed appointments or cancellations made with less than 24 hours' notice.
Letters / Forms (up to 2 pages) — $30
For letters or forms requiring clinician input.
Letters / Forms (3 or more pages) — $150
For extensive/comprehensive documents requiring clinician input.
Paper Statements — $5
Per requested paper billing statement.
